Unusual Page not found error..Need Help

i trying enable old survey form developed long time ago via else on cf 4. however, simple html form calls action cfm page , displays results. that's it. however, @ end of processing page, cf comes error of "file not found: /survey/index.cfm" there no index.cfm, obviously, none of code looking it, why if cf trying load it? attach entire cfm page simple processing, @ loss here...any appreciated. thank you.

do have application.cfm page looking it?
