Goofy little boxes

has run situation this? [rh x5.0.2]

occasionally, little box appears above every list item in topic file (i hope shows in attached code). never seems happen while editing file, apparently develops during generate/publish process (might residue of failed rltnnnn.htm review temp file?)

although happens rarely, it's embarassing have reviewers giggle @ results of glitch. fixing relatively simple, we'd prevent happening @ all.
